- Complete page 1 and page 2 of the application form.
- If you have questions or problems completing the application form, you may call us, send us an
e-mail, or come to our office.
- We can arrange for you to talk with a staff member in Chinese, French, German, Hindi, Italian,
Russian, and Spanish.
- Send your completed application form, together with original documents, one set of photocopies
and payment to:
